Sickness Absence

You should always call and notify your supervisor or manager before submitting this report. If you need to extend your absence, please use this form to submit an update. Please note: you don't need to hand in a separate self-certification. Filling in the first section (Report new sick leave) is enough as a self-certification.

A fit note is required from the 8th day of illness which you can hand in using the 2nd option in this form: "Hand in fit note" including a fit note and with the updated dates. It is always the latest report that applies.
